Position Overview:

The City of Toronto is seeking a dedicated and skilled Advanced Practice Nurse to provide exceptional primary health care services within a Long-Term Care Home. This role is pivotal in managing both acute and chronic health conditions, promoting wellness, and preventing diseases and injuries to ensure comprehensive health care delivery.

Key Responsibilities:

  • Administers primary health care to residents, focusing on the management of various medical conditions.
  • Conducts thorough health assessments, utilizing tailored assessment tools based on individual resident needs.
  • Documents comprehensive health histories, considering physical, psychosocial, emotional, cultural, and ethnic health dimensions.
  • Performs detailed physical examinations, identifying and interpreting both normal and abnormal findings.
  • Orders and analyzes diagnostic tests, applying evidence-based clinical reasoning to interpret results.
  • Integrates health information through critical inquiry and clinical reasoning to establish diagnoses and identify health risks.
  • Engages residents in discussions regarding health assessment findings, diagnoses, and prognoses, leveraging knowledge in various medical fields.
  • Selects suitable interventions from a spectrum of pharmacological and non-pharmacological options to enhance resident health and stability.
  • Ensures safe resident care by proactively addressing risks and mitigating potential harm.
  • Collaborates with the Medical Director and senior management to develop and implement evaluation processes aimed at improving clinical outcomes.
  • Works alongside community health services to promote continuity of care for residents.
  • Adheres to Accreditation Canada standards in daily practices.
  • Maintains resident files and health records in compliance with relevant legislation and organizational standards.
  • Identifies educational needs and collaborates with the Medical Director to develop training programs for the care team.

Essential Qualifications:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class.
  • Proven experience as a Nurse Practitioner in various care settings, including long-term and acute care.
  • Demonstrated expertise in providing compassionate care for elderly residents and working with frail older adults.
  • Certification in Geriatric Nursing or equivalent experience.

  • Strong independent judgment and problem-solving skills.
  • Leadership capabilities and experience working within inter-professional teams.
  • Proficient in providing clinical supervision and mentorship to students and other learners.
  • Effective communication skills, both verbal and written, with a keen attention to detail.
  • Competence in using computer software, including MS Office and web applications.
  • Ability to foster relationships with diverse stakeholders and work effectively in multidisciplinary teams.
  • Commitment to promoting equity, diversity, and respectful workplace culture.
  • Flexibility to work various shifts, including evenings and weekends.
  • Knowledge of relevant health legislation and regulations.
  • Willingness to enhance the dignity and reflect the diversity of residents through service delivery.
