Position Overview:

The City of Toronto is seeking a dedicated and skilled Advanced Practice Nurse to provide comprehensive health care services to residents in a Long-Term Care Home. This role is essential in managing both acute and chronic health conditions while promoting overall wellness and preventing disease.

Key Responsibilities:

  • Deliver primary health care services tailored to the unique needs of residents, focusing on both therapeutic management and health promotion.
  • Conduct thorough health assessments utilizing various tools and techniques, ensuring they are adapted to meet individual resident needs.
  • Document detailed health histories that encompass physical, psychosocial, emotional, cultural, and ethnic aspects of health.
  • Perform comprehensive physical examinations, accurately identifying and interpreting both normal and abnormal findings.
  • Order and analyze diagnostic tests, applying evidence-based clinical reasoning to interpret results.
  • Synthesize health information through critical inquiry to formulate diagnoses and assess health risks.
  • Effectively communicate assessment findings and diagnoses to residents, employing knowledge of relevant medical sciences.
  • Select and implement appropriate interventions, both pharmacological and non-pharmacological, to restore and maintain residents' health.
  • Ensure safe care practices by proactively addressing risks and mitigating potential harm to residents.
  • Collaborate with senior management and the Medical Director to develop and enhance evaluation processes aimed at improving clinical outcomes.
  • Engage with community health services to promote continuity of care across various health settings.
  • Maintain compliance with Accreditation Canada standards in daily operations.
  • Ensure accurate maintenance of resident files and health records in accordance with relevant legislation.
  • Identify educational needs and collaborate with the Medical Director to develop training programs for the care team.

Qualifications:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class.
  • Proven experience as a Nurse Practitioner in various care settings, including long-term care and acute care.
  • Demonstrated ability to provide compassionate care to elderly residents, particularly those in long-term care environments.
  • Certification in Geriatric Nursing or equivalent experience is preferred.
