Job Summary

Key Responsibilities
  • Conduct thorough assessments to determine eligibility for Home Care program supports
  • Develop and coordinate individualized care plans in collaboration with clients, families, and healthcare teams
  • Establish and maintain relationships with community partners, healthcare services, and local organizations
  • Provide education and support to clients and families on health management and self-care
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination
Requirements
  • Graduate of an approved Bachelor of Nursing education program with current CRNM registration
  • Two years of directly related community experience, preferably in Hospital Based Case Coordination
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations
  • Strong critical thinking and problem-solving skills, with ability to prioritize care and manage multiple demands
  • Proficiency in Windows-based computer programs, including Word, Excel, and Outlook
Preferred Qualifications
  • Canadian Community Health Nurses Certification (CCHN) or relevant applicable certification
  • Competence in Procura software and assessment tool interRAI-HC
  • Knowledge of Indigenous customs, traditions, and values, with ability to communicate effectively with Indigenous clients and families
Working Conditions

This is a full-time position, requiring availability to work days, evenings, and weekends. The successful candidate will be required to maintain a valid Manitoba Class 5 Driver's license and use a reliable motor vehicle suitable for all environmental conditions.
