Job Title: Case Coordinator

WRHA Community Health Services is seeking a skilled Case Coordinator to join our team. As a Case Coordinator, you will play a critical role in determining program eligibility and assessing the need for care at home or in a personal care home for the elderly or infirm adult.

Key Responsibilities:
  • Receive referrals and conduct assessments to determine client care needs related to Home Care.
  • Perform assessments and prioritize client needs.
  • Establish appropriate discipline coordinators and develop care plans.
  • Implement and coordinate care plans, including initiating medical rehabilitation and consultative services as necessary.
  • Provide professional intervention through counseling, teaching, and guidance.
  • Maintain current case counts and submit statistics.
  • Plan and organize work schedules and manage caseload demands effectively.
  • Utilize consultation and supervision.
  • Gather data on resources and resource needs related to caseload and community.
  • Participate in educational development and maintain current developments within own discipline.
Requirements:
  • Two years of directly related community experience preferred.
  • Previous experience as a Case Coordinator in Community Health Services preferred.
  • Experience with EFT implementation considered an asset.
  • Bachelor's degree in a related field required.
  • Valid driver's license and vehicle required.
  • Licensure/registration as per professional designation required.
