Case Manager

Role Summary: The Winnipeg Regional Health Authority is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in determining program eligibility and assessing care needs for the elderly or infirm adult. Your primary responsibility will be to coordinate the delivery of services based on established guidelines.
Key Responsibilities:
- Receive referrals and assess client and family care needs, prioritizing patterns and needs based on established criteria.
- Establish a discipline coordinator, develop care plans, and implement and coordinate care services in accordance with Winnipeg Regional Health Authority policies and procedures.
- Provide professional intervention, maintain current case counts, and plan work schedules to ensure efficient service delivery.
- Utilize consultation and supervision to gather data on resources and needs, and participate in educational development to enhance skills and knowledge.
- Establish and maintain liaison with local healthcare services and community resource networks to ensure comprehensive care.
Requirements:
- Two years of directly related community experience is preferred.
- Previous experience as a Case Coordinator in Community Health Services is preferred.
- A valid driver's license and vehicle are required.
- Licensure/registration as per professional designation is required.
Additional Requirements:
- Knowledge and experience with computers are required.
- Effective oral and written communication skills are essential.
