Job Summary

We are seeking a skilled Physiotherapist to join our team as a Home Care Specialist. As a key member of our interdisciplinary team, you will work closely with clients, families, and caregivers to provide comprehensive rehabilitation services and support individuals in achieving their goals and maintaining their independence.

Key Responsibilities

  • Assess and develop individualized rehabilitation care plans to support clients in achieving their goals and maintaining their independence.
  • Provide evidence-based physiotherapy interventions to clients in their homes, including assessment, treatment, and education.
  • Collaborate with interdisciplinary team members, including PT Assistants, to provide comprehensive care and support to clients.
  • Communicate effectively with clients, families, and caregivers to ensure seamless care coordination and transitions.
  • Participate in ongoing education and professional development to stay current with best practices and industry developments.

Requirements

  • Graduate of a recognized Post-Secondary program in Physical Therapy.
  • Current active registration and good standing as a Physiotherapist with the College of Physiotherapists of Ontario.
  • Previous experience in home care or rehabilitation settings an asset.
  • Valid driver's license and reliable vehicle required.
