Job Summary:

Closing the Gap Healthcare Group is seeking a skilled Physiotherapist to join our team in providing high-quality rehabilitation services to clients in their homes. As a Physiotherapist - Home Care Specialist, you will be responsible for assessing and developing individualized care plans to support clients in maintaining their independence and quality of life.

Key Responsibilities:

  • Conduct thorough assessments of clients' physical abilities and develop comprehensive care plans to address their rehabilitation needs.
  • Provide one-on-one therapy sessions to clients in their homes, focusing on improving their mobility, strength, and functional abilities.
  • Collaborate with a multidisciplinary team, including PT Assistants, to ensure seamless care coordination and effective communication with clients and their families.
  • Develop and maintain strong relationships with clients, their families, and caregivers to ensure a holistic approach to care.
  • Stay up-to-date with industry developments and best practices in rehabilitation therapy to continuously improve services and outcomes.

Requirements:

  • Graduate of a recognized Post-Secondary program in Physical Therapy.
  • Current active registration and good standing as a Physiotherapist with the College of Physiotherapists of Ontario.
  • Previous experience in Home Care or community-based rehabilitation settings is an asset.
  • Valid Driver's License and reliable vehicle for travel to client homes.
