About the Role:

  • Assess and develop personalized rehabilitation care plans to support clients in remaining at home and in their preferred environment.
  • Utilize creativity and critical thinking to provide complex preventative or corrective actions for clients to overcome obstacles and achieve their goals.
  • Collaborate with PT Assistants to support a diverse range of clients, including pediatric, adult, and senior care cases.
  • Partner with an interdisciplinary healthcare team to develop holistic treatment goals and service plans that prioritize client well-being.

Requirements:

  • Graduate of a recognized Post Secondary program in Physical Therapy.
  • Current active registration and good standing as a Physiotherapist with the College of Physiotherapists of Ontario.
  • Previous experience in Home Care is an asset.
  • Valid Driver's License and reliable vehicle for travel.
