Job Title: Social Worker

Medavie Health Services is a leading provider of primary health care solutions, emergency medical services, and community paramedicine programs across Canada.

We are seeking a skilled Social Worker to join our team in Listowel, Ontario. As a Social Worker, you will work collaboratively with our interdisciplinary team to provide mental health and addiction services to individuals and families in Huron, Perth, Bruce, and Wellington counties.

Key Responsibilities:

  • Conduct in-home client visits with the Mobile Mental Health Team.
  • Provide client support through Tele-Support access line.
  • Assess client's condition and presenting problem, and contributing factors.
  • Conduct mental health and risk assessments to facilitate client rapport and make referral recommendations.
  • Collaborate and share information with law enforcement and/or Paramedic Services.
  • Recommend and facilitate referrals to appropriate alternative resources as indicated.
  • Employ a technical knowledge of psychotropic medication and identify a need for further evaluation or consultation with physician/psychiatry.
  • Gather information from interviews with the client and collateral information from family and other professionals involved.
  • Use substantive knowledge of numerous areas, including mental illness, cognitive functioning, medical co-morbidity, and substance abuse to formulate clinical impressions and recommendations.
  • Ensure that evidence-based practices are embedded into client care.

Requirements:

  • Considerable clinical experience, preferably involving assessment of and exposure to acute as well as chronic and persistent mental illness.
  • Must be registered and in good standing with the Ontario College of Nurses.
  • Demonstrated ability to provide initial psychological interviews and assessments.
  • Experience on interdisciplinary teams, in program development and case management.
  • Requires independent critical thinking, clinical judgment, and independent decision-making skills to develop, implement, and monitor care plans.
  • Must have the ability to work effectively as a reliable member of a team, work independently with minimal supervision, and adapt to a changing, evolving environment.
  • Must have a valid Driver's license in good standing.
  • Applicants must have a good previous work attendance record.
  • The successful applicant must provide a satisfactory criminal record and vulnerable sector check prior to beginning employment.
