Nova Scotia Health Authority is seeking a compassionate and dedicated Community Outreach Worker to join our team.

As a Community Outreach Worker, you will provide essential clinical services to adults and their families facing challenges related to mental illness, substance use, or gambling disorder. Your responsibilities will encompass screening, assessment, brief intervention, case management, and individual and group treatment sessions. You will also play a crucial role in guiding individuals through available resources within Mental Health and Addictions and the broader community.

Responsibilities:
  • Conduct comprehensive screenings and assessments of clients.
  • Provide brief interventions and support to individuals experiencing mental health or substance use challenges.
  • Develop and implement individualized case management plans to address client needs.
  • Facilitate individual and group therapy sessions, utilizing evidence-based practices.
  • Advocate for clients and connect them with appropriate community resources and services.
  • Maintain accurate and up-to-date client records.
Qualifications:
  • Bachelor of Social Work degree
  • Registration or eligibility for registration with the relevant professional regulatory body
  • One year of post-Bachelor of Social Work clinical experience in Mental Health and Addictions
  • Experience in screening, risk assessment, and suicide risk assessment
  • Knowledge of DSM, harm reduction, and recovery-oriented practice
  • Strong communication, problem-solving, and interpersonal skills
  • Proficiency in motivational interviewing, group facilitation, and crisis support
  • Demonstrated ability to make sound clinical decisions and work independently
  • Proficiency in French is preferred
