Position Overview

Shared Health is dedicated to enhancing the integration of patient-centered clinical and preventive health services across Manitoba. Our mission is to provide a comprehensive range of mental health services that support recovery and promote well-being for all residents.

The PACT Service Coordinator plays a crucial role in the assessment, planning, and coordination of treatment and rehabilitation services for individuals with serious psychiatric conditions. This position operates under the clinical supervision of the team leader and psychiatrist, ensuring that services are delivered in accordance with the principles of psychosocial rehabilitation.

Key Responsibilities
  • Assess and develop individualized treatment plans for program participants.
  • Coordinate and facilitate support services in collaboration with a multidisciplinary team.
  • Act as a shift manager or specialist in areas such as substance abuse, vocational support, or spiritual care as designated by the team leader.
Qualifications
  • A minimum of four years of relevant experience in community settings, particularly with individuals facing persistent psychiatric challenges.
  • A degree in a related health or human services field is required.
  • Proof of registration with the appropriate regulatory body is essential.
Skills and Competencies
  • In-depth knowledge of psychosocial rehabilitation principles.
  • Strong skills in service assessment, planning, and coordination.
  • Experience working with individuals dealing with addiction issues is advantageous.
  • Excellent networking, organizational, and communication skills.
