Primary Care Nurse

We are seeking a highly skilled and dedicated Registered Nurse to join our Primary Care Team at Northern Health Authority. As a Primary Care Nurse, you will play a vital role in providing compassionate patient care and working collaboratively with our interprofessional team to deliver high-quality services to our patients.
Key Responsibilities- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Develop and implement individualized care plans in collaboration with the interprofessional team.
- Conduct assessments and provide education to patients, families, and the general public on health-related topics.
- Collaborate with community partners and healthcare providers to promote health and wellness in our communities.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid BC Driver's License.
- Experience working in a primary care setting and developing and implementing individualized care plans.
- Knowledge of adult learning principles and facilitation skills to effectively educate patients and families.
- Ability to work collaboratively with an interprofessional team and communicate effectively with patients, families, and healthcare providers.
