Northern Health is seeking a dedicated and compassionate Social Worker to join our dynamic Primary Care team. In this role, you will utilize your expertise to provide comprehensive support to individuals, families, and communities within our healthcare system.
As an integral member of the interprofessional team, you will collaborate with physicians, nurses, and other healthcare professionals to deliver person-centered care that addresses the diverse needs of our patients.
Responsibilities- Conduct comprehensive assessments to identify social, emotional, and environmental factors impacting patient well-being.
- Develop individualized care plans in collaboration with patients, families, and the healthcare team.
- Provide counseling, support, and advocacy services to individuals and families facing various challenges.
- Connect patients with community resources and support networks to enhance their overall health and well-being.
- Collaborate effectively with interdisciplinary teams to ensure seamless coordination of care.
- Promote a culture of person-centered care that values diversity, equity, and inclusion.
We are seeking a highly motivated individual with a Master's degree in Social Work (MSW) from an accredited program. A valid license to practice social work in British Columbia is required.
Proven experience working in a healthcare setting, particularly within primary care, is highly desirable. Excellent communication, interpersonal, and problem-solving skills are essential for success in this role.
