Primary Care LPN Opportunity

We are seeking a motivated Licensed Practical Nurse to join our Primary Care team in Terrace, BC. As a Primary Care LPN, you will be an integral part of our interprofessional team, providing compassionate patient care and working collaboratively with healthcare professionals to deliver high-quality services.

About the Role

The Primary Care LPN will provide clinical follow-up according to the person's primary Care Plan and personal goals of care. You will work closely with the interprofessional team to develop and implement plans of care, supporting people and their families to achieve and maintain optimal health.

Responsibilities
  • Perform assessments, plans, and implements personal and nursing care to people
  • Administer oral, subcutaneous, and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures
  • Work collaboratively with the interprofessional team to provide comprehensive care
  • Communicate effectively with people and their families, establishing and maintaining rapport
  • Observe and recognize changes in a person's condition or health status, planning and implementing a plan of care as needed
Requirements
  • Graduation from a recognized program for Practical Nurses with one year of recent, related experience
  • Registration with BC College of Nurses and Midwives as a practicing LPN registrant
  • Current, valid BC Driver's License
