Transitional Care Coordinator

Job Summary:
We are seeking a skilled Transitional Care Assistant to join our team at the Canadian Red Cross. As a key member of our care team, you will play a vital role in supporting clients in their transition from hospital to home.
Key Responsibilities:
- Facilitate client transportation home using local services and/or agency vehicles.
- Support clients in understanding and implementing hospital discharge instructions.
- Complete errands to meet clients' physical needs, such as nutrition, medication, and medical supplies.
- Plan and arrange home management duties, including cooking, cleaning, and laundry.
- Facilitate client and caregiver engagement, providing information on client-related issues.
- Communicate recommendations and concerns to clients, caregivers, and the circle of care.
- Provide companionship to seniors suffering from social isolation.
- Complete risk assessments and evaluate clients' risk of falls.
- Support clients post-discharge to remove barriers and reduce risk in accessing resources.
Requirements:
- Minimum two years post-secondary education in social services, gerontology, or a related field.
- Three years of job-related experience or an equivalent combination of education and experience.
- Fluency in English and French is required.
- Excellent computer skills, including Microsoft Office.
- Valid driver's license and access to a reliable vehicle.
Working Conditions:
- The work is performed in a combination of office, hospital, community, and client home settings.
- The role requires physical bending, stretching, and repetitive motions.
