Position Overview: The Housing Outreach Coordinator plays a crucial role in addressing homelessness within the community. This position is primarily responsible for the execution of the Supporting Individuals and Families to Transition (SHIFT) Housing Focused Outreach initiative. The Coordinator will operate under the guidance of the Executive Director of Fresh Start Services, ensuring effective outreach to individuals experiencing homelessness and those at risk.

Key Responsibilities: The Housing Outreach Coordinator is dedicated to diminishing homelessness in the area by performing essential tasks related to client engagement and access to community resources. This includes working closely with individuals and families to explore housing options and support systems.

Core Duties: The Coordinator is tasked with advancing the mission of Fresh Start Services by providing outreach services that connect clients with necessary community supports, including the By Names List. Responsibilities include:

  1. Identifying and engaging with individuals experiencing homelessness through direct outreach.
  2. Conducting intake assessments and facilitating referrals to appropriate services within the Coordinated Access System (CAS).
  3. Offering ongoing case management to assist clients in achieving housing readiness.
  4. Maintaining a non-judgmental and trauma-informed approach in all interactions.
  5. Collaborating with community partners to enhance outreach efforts and resource availability.

Client Engagement: The Coordinator will actively reach out to individuals in various settings, including public spaces and encampments, to build rapport and facilitate access to services. This role requires effective communication and the ability to provide crisis management and support.

Data Management and Reporting: The Housing Outreach Coordinator is responsible for accurate data collection and reporting to ensure clients are effectively tracked within the By Names List. This includes maintaining detailed case notes and statistical data for organizational reporting.

Work Environment: Fresh Start Services promotes a flexible and inclusive workplace culture, emphasizing harm reduction and trauma-informed practices. Successful candidates will demonstrate a commitment to these principles while engaging with clients.

Job Type: Full-time

Compensation: $23.00 per hour

Schedule: Monday to Friday, with occasional weekend work as needed.

Qualifications: A secondary school education is preferred.
