Vancouver Coastal Health is seeking a skilled Physiotherapist to join our Rehabilitation Services team in the Sunshine Coast Home Care Services. As a Physiotherapist with VCH, you will work in partnership with clients and families to establish goals and provide physiotherapy services in an interprofessional team model.
Key Responsibilities- Assess service eligibility and develop, implement, and adapt care plans for clients.
- Deliver direct care and provide care management to assist clients in managing their own care.
- Work in consultation with other system partners and incorporate current evidence into practice.
The District Municipality of Sechelt is a charming seaside community located on the lower Sunshine Coast of British Columbia. It offers a relaxed lifestyle with easy access to outdoor activities, including hiking, biking, and boating.
Qualifications- Current full registration with the College of Physical Therapists of BC (CPTBC).
- Two (2) years' recent, related experience in home and community care.
- Valid BC Driver's License and access to a personal vehicle required for local travel.
