Physiotherapist

Vancouver Coastal Health is seeking a skilled Physiotherapist to join our Rehabilitation Services team in the Sunshine Coast Home Care Services, serving Gibsons, Sechelt, and Pender Harbour in BC.

• Work in partnership with clients and families to establish safe, realistic, and reasonable goals, assessing, planning, coordinating, organizing, implementing, evaluating, and transitioning physiotherapy and other services in an interprofessional team model.
• Assess service eligibility, facilitate the development, implementation, and adaptation of a care plan, deliver direct care, and provide care management to assist clients to manage their own care and navigate through the various services available within the community setting.
• Work in consultation with other system partners and incorporate current evidence into practice.

The District Municipality of Sechelt is a charming seaside community located on the lower Sunshine Coast of British Columbia, approximately 50 km northwest of Vancouver. It offers a unique blend of natural beauty, outdoor activities, and a relaxed lifestyle.

• Current full registration with the College of Physical Therapists of BC (CPTBC).
• Two (2) years' recent, related experience in home & community care.
• Valid BC Driver's License & access to a personal vehicle required for local travel.
