About the Role
Vancouver Coastal Health is seeking a skilled Nurse Case Manager to join our Home Health team at Pacific Spirit Community Health Centre in Vancouver, BC.
Key Responsibilities
- Manage a caseload of clients with complex health needs, providing comprehensive care and support.
- Conduct assessments, develop care plans, and coordinate services to meet client needs.
- Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals.
- Provide education and support to clients and their families, promoting independence and self-management.
- Develop and maintain formal partnerships with General Practitioners and other healthcare providers.
About Home Health Programs
Home Health programs provide a range of healthcare and support services to individuals living in the community with acute, chronic, palliative, or rehabilitative healthcare needs.
Qualifications
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Three years' recent clinical nursing experience, preferably in acute care or home care.
- Valid BC Driver's License and reliable transportation.
