Nurse Case Manager

Job Summary
Vancouver Coastal Health is seeking a skilled Nurse Case Manager to join our Assisted Supportive Living team in Vancouver, BC.
As a Nurse Case Manager, you will be responsible for managing a select population of eligible clients who are chronically ill or have long-term physical disabilities.
Key responsibilities include:
- Conducting assessments, identifying problems, and addressing issues or variances from the plan of care in collaboration with the interdisciplinary team and community agencies.
- Providing continuity of care across the continuum by liaising with interdisciplinary team members, coordinating clinical interventions, and evaluating client outcomes.
- Developing and maintaining formal General Practitioner Partnerships as appropriate and in accordance with CHA service delivery model.
- Acting as a clinical resource for the interdisciplinary team by providing information based on nursing theory and practice related to clients with complex healthcare needs.
- Consulting, conferring, and collaborating with multidisciplinary team members, physicians, educational resources, and other healthcare professionals to achieve excellence in client-centered education and research.
Qualifications
Required qualifications include:
- Current practicing registration as a Registered Nurse or a Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years' recent clinical nursing experience working in acute care with a medical and/or geriatric focus, recent case management or home care nursing experience, or an equivalent combination of education, training, and experience.
- Local area travel requires the use of a personal vehicle.
- Valid BC Driver's License.
