Nurse Case Manager Position

Vancouver Coastal Health is seeking a Registered Nurse to join the team at Lions Gate Hospital in North Vancouver, BC. As a Nurse Case Manager, you will be responsible for managing a select population of eligible Assisted Living clients. Your duties will include determining client goals and needs, assessing client situations, and working effectively with community resources to plan care and support in accordance with their health needs.
Key Responsibilities:
- Manage a select population of eligible Assisted Living clients.
- Determine client goals and needs, assess client situations, and work effectively with community resources.
- Plan care and support in accordance with client health needs.
- Work within a multidisciplinary team context to provide continuity of care across the continuum.
- Act as a clinical resource for the interdisciplinary team.
Qualifications:
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years' recent clinical nursing experience in Home Health and case management or an equivalent combination of education, training, and experience.
- Local area travel requires the use of a personal vehicle. Valid BC Driver's License.
