Under the guidance of the Casework Supervisor, the Remote Support Specialist is primarily tasked with managing and executing all facets of remote case management, including Intake, Needs Assessment, and Casework. This role involves providing support, information, education, and referrals to clients while fostering a relationship that promotes client autonomy. The Remote Support Specialist will serve clients in BC, Alberta, and other western provinces as necessary.

KEY RESPONSIBILITIES

CASE MANAGEMENT

  1. Intake & Needs Assessments
  • Evaluate potential clients for intake eligibility as directed by the Casework Supervisor.
  • Engage in the selection and admission process for clients into suitable programs.
  • Review client histories, conduct interviews to gather case details, and prepare intake documentation.
  • Perform a comprehensive review of documents and application forms to ensure completeness.
  • Assess immediate client needs and document action plans within the CRM system.
  • Inform clients of available options and resources tailored to their needs.
  • Guide clients in formulating and executing a six-month action plan.
  • Provide essential support to clients to help achieve their action plans.
  1. Referrals
  • Compile potential resources for clients located on the west coast.
  • Offer informed referrals based on specific client needs and assist during the connection process.
  1. Case Closure
  • Encourage clients to reach their goals, including sustainable housing, employment, education, or financial assistance.
  • Assist clients in accessing various community or governmental programs related to housing, welfare, childcare, and education.

CLIENT SUPPORT AND DEVELOPMENT

  • Maintain regular communication with clients to evaluate their progress, provide emotional support, and assist with goal setting and problem-solving.
  • Advocate for clients' needs with other community and governmental organizations.
  • Keep a daily log of client interactions and activities.

REPORTING

  • Provide ongoing updates regarding client progress to the casework supervisor.
  • Maintain accurate case notes, client records, documentation, and statistical data.
  • Participate in meetings to discuss transitions for clients moving out of the program.
  • Engage in monthly casework meetings and national staff meetings as required.

AD HOC TASKS

  • Assist with intakes on a regular basis as needed for various teams.

QUALIFICATIONS AND EXPERIENCE

  • Degree or Diploma in a relevant human/social service field such as Social Work or Child and Youth Care Work, along with one (1) year of recent related experience, or an equivalent combination of education and experience.
  • Familiarity with relevant policies, procedures, and legislation.
  • Current First Aid/CPR Certification is required.
  • Clear Criminal and Vulnerable Sector Police Clearance.
  • Awareness of personal triggers and commitment to personal wellness.
  • Understanding of the stresses associated with the position and adherence to a self-care action plan.
  • Respect for clients' rights to their own values, beliefs, culture, and choices.
  • Empathetic and client-centered approach when working with women and children.
  • Understanding of the cycle of abuse and the dynamics of power and control.
  • Strong interpersonal skills and ability to engage with individuals from diverse cultural backgrounds.
