This is a challenging and rewarding opportunity for a Registered Practical Nurse to join our team at ParaMed Home Health Care. As a Community Nursing Role, you will be responsible for providing high-quality nursing care to patients in their homes, working collaboratively with our team of healthcare professionals to ensure the best possible outcomes.
Key Responsibilities
- Participate in scheduled visits in the community to assess and address patient needs within your scope of practice
- Provide safe and effective nursing care, using appropriate assessment tools and techniques to identify individual patient needs
- Develop and implement care plans, working collaboratively with patients and their families to achieve optimal health outcomes
- Engage patients in their care, using a collaborative approach to build rapport and trust
- Maintain accurate and up-to-date patient records, reflecting patient encounters and care plans
Requirements
- Registration with the College of Nurses of Ontario as a Registered Practical Nurse in good standing
- Valid driver's license and access to a reliable vehicle
- Experience in delivering community care, an asset
- Reliability, flexibility, and a strong commitment to patient-centered care
- Ability to work independently, with minimal supervision
