Key Responsibilities

  • Participate in scheduled visits in the community to assess and address patient needs within your scope of practice

  • Develop and implement care plans, working collaboratively with patients and their families to achieve optimal health outcomes
  • Engage patients in their care, using a collaborative approach to build rapport and trust
  • Maintain accurate and up-to-date patient records, reflecting patient encounters and care plans

Requirements

  • Registration with the College of Nurses of Ontario as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a reliable vehicle
  • Experience in delivering community care, an asset
  • Reliability, flexibility, and a strong commitment to patient-centered care
  • Ability to work independently, with minimal supervision
