Job Summary:

Kahnawake Shakotiia'takehnhas Community Services is seeking a highly skilled and experienced individual to join our team as a Case Worker. As a Case Worker, you will be responsible for providing counseling and support to clients and families of Assisted Living Services.

Key Responsibilities:

  • Provide counseling and support to clients and families of Assisted Living Services
  • Work with clients and families living with developmental delays and/or mental health issues
  • Conduct intake services, assessments, and reassessments
  • Develop and maintain individual service plans
  • Work with families and within an interdisciplinary team setting

Requirements:

  • University degree in social work with one to three years of work-related experience in the field of Special Needs and/or Mental Health
  • College program in social work or other social science or health-related discipline, with three to five years of previous work experience in the field of Special Needs and/or Mental Health
  • Experience and working knowledge conducting assessments, evaluations, and completing Integrated Service Plans
  • A valid driver's license and access to a vehicle

Working Conditions:

  • Indefinite full-time position with a 6-month probationary period
  • 35 hours per week; must be willing to work flexible hours as necessary

Compensation:

  • Starting salary: $31.75 per hour
