Compassionate Primary Care Nurse

Are you passionate about providing quality nursing care? We are looking for a committed Registered Nurse who is eager to deliver empathetic patient support in a Primary Care setting.
In alignment with Northern Health's vision for an integrated service system rooted in Primary Care, the Primary Care Nurse (PCN) adheres to the professional standards and ethical guidelines established by the BC College of Nurses and Midwives (BCCNM). The PCN collaborates within an interprofessional team to implement best practices, ensuring safe, competent, and ethical care for individuals and families across various environments such as clinics, homes, educational institutions, and community settings. The role involves delivering clinical follow-up in accordance with the individual's Primary Care Plan and personal care objectives, developed in partnership with the healthcare team.
Work Schedule:Monday to Friday, Day Shift - 08:00 to 16:30
CompensationThe updated wage is: $55.91/hour.

- Current registration with the BC College of Nurses and Midwives as a practicing RN or RPN.
- A minimum of three years of recent clinical nursing experience in primary care or chronic disease management, including experience in patient education.
- A valid B.C. Driver's License.
- Assessment and Intervention: Proven ability to conduct thorough client assessments and deliver nursing care through prescribed interventions.
- Communication: Strong communication skills to effectively engage with clients, families, and interdisciplinary team members.
- Critical Thinking: Ability to analyze and synthesize data from various sources to solve problems effectively.
- Person and Family-Centered Practice: Commitment to promoting care that respects individual and family needs within their community context.
- Teaching and Mentoring: Knowledge of adult learning principles to educate patients and support the learning of peers.
- Management: Efficiently manages time and resources to enhance service delivery and team collaboration.

- Knowledge Integration: Utilizes factual information and best practices to inform decisions and actions.
