We are seeking a highly motivated and dedicated Registered Nurse to join our team in Primary Care at Northern Health Authority. As a Primary Care Nurse, you will play a vital role in delivering compassionate patient care and working collaboratively with our interprofessional team to provide high-quality services to our patients.
Key Responsibilities- Provide direct clinical care and services to individuals, groups, and communities in Primary Care settings.
- Develop and implement care plans in collaboration with the interprofessional team, taking into account the patient's primary care plan and personal goals of care.
- Work effectively with patients, families, and healthcare teams to promote person-centered care and improve health outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
- Collaborate with other healthcare professionals to provide comprehensive care and support to patients and their families.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid BC Driver's License.
- Experience in developing and providing formal education to patients, families, and the general public.
- Knowledge of adult learning principles and facilitation skills to effectively educate patients and others about health care and well-being.
