We are seeking a highly motivated and compassionate Registered Nurse to join our team at Northern Health Authority. As a Primary Care Nurse, you will play a vital role in delivering high-quality patient care and promoting health and well-being in our community.
Key Responsibilities- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Work collaboratively with the interprofessional team to provide comprehensive care and services to individuals, groups, and communities.
- Develop and implement care plans that address the unique needs and preferences of patients and their families.
- Provide education and support to patients and their families to promote health and well-being.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in patient care.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid driver's license.

- Four weeks vacation with one year of continuous service.
