About the RoleThe Nurse Practitioner will be responsible for assessing patients' health needs, developing and implementing individualized care plans, and providing education and support to patients and their families. You will work collaboratively with a multidisciplinary team to ensure seamless care transitions and optimal patient outcomes.
Responsibilities- Conduct comprehensive patient assessments, including physical examinations and diagnostic testing
- Develop and implement individualized care plans, taking into account patients' medical, social, and cultural needs
- Provide education and support to patients and their families, promoting self-management and empowerment
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM)
- Recent relevant clinical nursing experience
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Ability to work independently and as part of a team, with a focus on patient-centered care
- Strong problem-solving and critical thinking skills, with the ability to analyze complex patient situations and develop effective solutions
