Join Vancouver Coastal Health (VCH) as an EDiCare Physician

The EDiCare initiative is a collaborative Geriatrics program established within the Emergency Department of Vancouver General Hospital (VGH). The program's mission aligns with the "Home is Best" philosophy for the Frail Elderly, aiming to divert frail seniors from hospital admissions towards safe discharges back home, supported by community and medical resources.

The role of the EDiCare Physician offers a distinctive opportunity for medical professionals dedicated to serving the growing population of frail elderly individuals. As part of a well-established interdisciplinary team within the Emergency Department at VGH, the EDiCare Physician focuses on planning safe discharge options. Patients in the Emergency Department are assessed for their suitability for discharge home, rather than facing extended hospital stays that can negatively impact their quality of life.

Responsibilities of the EDiCare Physician at Vancouver General Hospital (VGH):

Develop and execute safe discharge strategies in partnership with acute care services, community primary care providers, and Home Health services. Deliver patient and family-centered care. Assist the Emergency Department in managing complex geriatric patients to prevent unnecessary hospital admissions. Foster collaborative and trusting relationships with interdisciplinary team members, health authority personnel, and community physicians. Coordinate with relevant community primary care providers and specialists to facilitate safe discharge plans for frail elderly patients. Maintain clear communication with patients, families, and other healthcare professionals. Engage patients and family members in meaningful Goals of Care discussions. Provide medical oversight for home health nursing staff caring for patients in the community. Collaborate with Home and Community Health to promote team-based care, aiming to reduce unnecessary emergency visits and hospital admissions.

Qualifications

Education, Licensing, & Experience :

Current Full License from the College of Physicians of BC (CPSBC). Membership with the Canadian Medical Protective Association (CMPA) – Mandatory. Certification from the Canadian College of Family Practitioners (CCFP) – Mandatory.

Knowledge & Abilities:

Interest or training in Geriatrics. Commitment to the "Home is Best" care philosophy. Strong communication skills. Experience working effectively as part of an interdisciplinary team.
