Join Vancouver Coastal Health (VCH) as an EDiCare Physician
The EDiCare initiative is a collaborative Geriatrics program established within the Emergency Department of Vancouver General Hospital (VGH). This program is guided by the principle that "Home is Best" for the Frail Elderly, aiming to facilitate safe discharges home rather than unnecessary hospital admissions.
The role of the EDiCare Physician offers a distinctive opportunity for medical professionals dedicated to serving the frail elderly demographic. As part of a well-established interdisciplinary team in the Emergency Department at VGH, the EDiCare Physician focuses on effective discharge planning. Patients who are frail and present in the Emergency Department are assessed for their suitability for safe discharge to their homes, thereby minimizing the risks associated with extended hospital stays.
Responsibilities of the EDiCare Physician at Vancouver General Hospital (VGH):
Develop and execute safe discharge strategies in partnership with acute care services, community primary care providers, and Home Health services. Deliver patient and family-centered healthcare. Assist the Emergency Department in managing complex geriatric cases to prevent unnecessary hospital admissions. Foster collaborative and trusting relationships with interdisciplinary team members, health authority personnel, and community physicians. Coordinate with relevant community primary care providers and specialists to ensure safe discharge plans for frail elderly patients. Maintain clear communication with patients, families, and other healthcare professionals. Engage patients and their families in meaningful discussions regarding Goals of Care. Provide medical oversight for home health nursing staff caring for patients in the community. Collaborate with Home and Community Health to promote team-based care and reduce avoidable Emergency Room visits and admissions.Qualifications

Current College of Physicians of BC (CPSBC) License - Full Canadian Medical Protective Association (CMPA) – Mandatory Canadian College of Family Practitioners (CCFP) – MandatoryKnowledge & Abilities:
Interest or training in Geriatrics Commitment to the "Home is Best" care philosophy Strong communication skills Experience working effectively as part of an interdisciplinary teamClosing Statement
