Clinical Case Coordinator

Hunter's House is a pioneering mental health program offering comprehensive, evidence-based treatment to young people in transition. Our short-term assessment and stabilization program is designed for individuals aged 12-18 who require a supportive environment to stabilize and thrive. Our team of multi-disciplinary professionals provides 24/7 care, focusing on meeting each individual's basic needs and delivering psychiatric and psychological support for long-term treatment.

We are seeking a skilled Clinical Case Coordinator to join our team. The ideal candidate will possess:

  • Excellent communication and interpersonal skills
  • Sound judgment and decision-making abilities
  • Conflict resolution and crisis management skills
  • Experience working with youth and establishing therapeutic relationships
  • Proficiency in computer software applications

Responsibilities:

  • Conduct assessments, review clinical notes, and develop plans of care
  • Collaborate with the Hunter House team to implement plans of care and provide support to youth
  • Engage with families and external partners as needed
  • Requires registration with a professional college or a relevant master's degree
