Clinical Care Coordinator

We are seeking a highly skilled Clinical Care Coordinator to join our team at Southlake Regional Health Centre. As a key member of our Regional Cardiac Program, you will play a vital role in ensuring the efficient flow of patient care activities.
Key Responsibilities:
• Coordinate patient flow activities across the Regional Cardiac Care Program in collaboration with the Triage coordinators and PFN/Charges of the cardiac program.
• Oversee daily staffing, ensure quality of patient care delivery, and patient flow related activities within the program.
• Lead the team in implementing evidence-based practices that are sensitive to limited resources within a patient-centred care philosophy.
• Collaborate with the manager regarding human resource responsibilities, including recruitment, just-in-time teaching, and ongoing performance evaluation.
• Liaise with members of the inter-professional team to ensure excellence in patient care through monitoring of standards of care and patient outcomes.
Qualifications:
• RN Certificate of registration from the College of Nurses of Ontario.
• Baccalaureate Degree in Nursing Science (preferred).
• Minimum of 2-5 years recent and relevant cardiac care experience.
• Demonstrates knowledge in clinical practice in the Cardiac Program.
• Demonstrates the ability to communicate and work with all members of the inter-professional team.
• Demonstrates the ability to problem-solve clinical issues with the support of the manager.
• Demonstrates a strong patient and customer-focused philosophy in all interactions.
• Possesses the ability to implement strategies that effectively facilitate change.
• Demonstrates clinical decision-making and organizational skills.
• Demonstrated excellence in communication, both written and verbal.
• Demonstrated problem-assessment skills.
• Demonstrated commitment to customer service and teamwork.
• Demonstrated individual leadership skills.
