Job Summary:

The Activity Instructor II (AI2) is a key member of the Prairie Mountain Health team, responsible for providing mobile community-based services to individuals with mental health problems. This role requires a strong therapeutic relationship with clients, assessment of individual strengths and needs, and development of personalized plans to promote empowerment, independence, and self-responsibility.

Key Responsibilities:

  • Engage with clients to develop a therapeutic working relationship and assess individual strengths, interests, goals, barriers to goal achievement, functional and resource needs.
  • Develop a strength-based plan with the client that promotes empowerment, independence, and self-responsibility, and work in collaboration with the client and their support network.
  • Implement the activity plan, evaluate the client's performance in selected tasks, and provide constructive feedback that promotes personal growth.
  • Provide ongoing teaching, training, guidance, and direct feedback in the implementation of a client plan, and monitor and modify the client's plan to accommodate fluctuations in functioning and/or extenuating circumstances.
  • Develop and post an activity schedule as per program requirements to enhance client participation in planned activities, and evaluate and re-prioritize the activity schedule in accordance with patient needs.
  • Participate in multi-disciplinary planning, communicate with team members, and assume responsibility for client assignments, and consult with Occupational Therapist, Community Mental Health Worker, and/or additional team members regarding activities for patients with specific functional impairments.

Group Facilitation and Community Engagement:

  • Work with individuals and small groups to identify learning and working needs, and develop individual and small group awareness of resources through information sharing and support.
  • Work with community agencies to modify resources to accommodate individual needs, and reduce gaps and barriers to service, and link individuals and small groups to community agencies and resources to assist in the achievement of their personal goals.
  • Identify and provide opportunities for expanded life experiences to increase self-confidence and self-esteem with the goal of recovery, and organize and implement group activities.
  • Provide instruction for selected tasks to develop knowledge and abilities, and act as a consultant for job stations and off-sight working locations as appropriate.

Health Promotion/Disease Prevention:

  • Participate in regular planning to organize and deliver programs that promote health, and communicate with clients, internal and external partners to increase awareness, knowledge, and understanding of mental health issues.
  • Plan and deliver health-promoting activities with clients, families, and internal and external partners, and participate in mental health promotion through education and public relations activities for clients, families, community groups, and organizations.
  • Implement programming aimed at preventing further disability, fostering independence, and enhancing quality of life in the least intrusive manner possible.

Crisis Management:

  • Facilitate ongoing client education and training related to safe work practices, and mediate risk and address safety concerns for clients within the environment.
  • Assess the potential for crisis situations and intervene to de-escalate/defuse situations, and engage the client and relevant others in the development and documentation of a work-related crisis plan.
  • Problem solve and provide feedback to involved parties following each crisis situation.

Program-Related Responsibilities:

  • Maintain activity program statistics and provide monthly reports, and record assessments, activity plans, and evaluations appropriately as per program requirements.
  • Ensure a sufficient inventory of supplies are available to meet the activity program requirements by maintaining records of supplies used, and requisitioning for replacements as needed.
  • Ensure equipment is used safely, stored appropriately, and maintained in good working order, and in conjunction with manager/supervisor prepare area budget and reports anticipated variances.
  • Advises the Program Manager of recommendations for capital budget expenditures, and maintains confidentiality in all matters pertaining to patients and their families.
  • Prioritize workload by utilizing sound organizational skills, and recognize legal issues in relation to the treatment of patients, in particular the provision of the Mental Health Act and Protection for Persons in Care Act.
  • Assist other Mental Health Programs when necessary to meet clinical and operational needs, and perform other duties as assigned.
