Activity Therapist

Job Summary:
The Activity Instructor II (AI2) is a key member of the Prairie Mountain Health team, responsible for providing mobile community-based services to individuals with mental health problems. This role involves assessing, developing, and implementing individual and group programming to engage clients with their environment.
Key Responsibilities:
Client Engagement and Support:
- Develop a therapeutic working relationship with clients to identify their strengths, interests, goals, and barriers to goal achievement.
- Assess individual functional and resource needs in the living, learning, working, and social domains of functioning.
- Collaborate with clients and their support network to develop a strength-based plan that promotes empowerment, independence, and self-responsibility.
- Implement the activity plan, evaluate client performance, and provide constructive feedback to promote personal growth.
Group Facilitation and Community Engagement:
- Work with individuals and small groups to identify learning and working needs, and develop individual and small group awareness of resources.
- Link clients to community agencies and resources to assist in achieving their personal goals.
- Organize and implement group activities to increase self-confidence and self-esteem.
Health Promotion and Disease Prevention:
- Participate in regular planning to organize and deliver programs that promote health and well-being.
- Communicate with clients, internal and external partners to increase awareness, knowledge, and understanding of mental health issues.
Crisis Management:
- Facilitate ongoing client education and training related to safe work practices.
- Mediate risk and address safety concerns for clients within the environment.
- Assess the potential for crisis situations and intervene to de-escalate/defuse situations.
Program-Related Responsibilities:
- Maintain activity program statistics and provide monthly reports.
- Record assessments, activity plans, and evaluations appropriately, as per program requirements.
- Ensure a sufficient inventory of supplies are available to meet the activity program requirements.
Requirements:
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and internal and external partners. A strong understanding of mental health issues and community resources is essential, as is the ability to work in a fast-paced environment and prioritize tasks effectively.
