Job Summary

The Salvation Army is seeking a highly skilled and compassionate Outreach Case Worker to join our team. As a key member of our outreach team, you will provide critical support and supervision to clients, ensuring their safety and well-being.

Key Responsibilities
  • Client Support and Supervision
    • Monitor the environment and clients to ensure safety and security
    • Develop and implement case management plans for assigned clients
    • Provide emotional support and advocacy as needed
  • Documentation and Reporting
    • Maintain accurate and up-to-date documentation of client interactions and progress
    • Prepare and submit reports to management and funders
  • Team Collaboration
    • Participate in case conferences and team meetings
    • Collaborate with colleagues to ensure seamless client support
  • Administrative Tasks
    • Perform intake and discharge procedures for clients
    • Collect and record case management statistics
Requirements
  • Education and Experience
    • Completion of more than two years of Community College
    • Post-Secondary Education or equivalent in Social Work or a related field
    • Minimum one year of prior related experience in a social services environment
  • Skills and Abilities
    • Strong communication and interpersonal skills
    • Ability to work independently and as part of a team
    • Self-motivated and disciplined
  • Certifications and Licenses
    • Certified in First Aid/CPR and Non-Violent Crisis Intervention
    • Valid Ontario Driver's License and personal vehicle
Working Conditions

This is a permanent full-time position with 40 hours per week. Flexibility in scheduling is required, and the work schedule may vary according to the needs of the position.
