Job Summary

Halton Healthcare is seeking a skilled Clinical Social Worker to join our Critical Care team. As a key member of our interprofessional team, you will provide comprehensive psychosocial support to patients and their families, ensuring a safe and supportive transition back to the community or institutional setting.

Key Responsibilities
  • Conduct thorough social work and psychosocial assessments, utilizing a variety of modalities and adhering to professional standards and clinical policies.
  • Develop and implement individualized care plans, coordinating with the healthcare team to ensure seamless transitions.
  • Participate in team conferences, educational rounds, and interdisciplinary discussions to promote collaboration and best practices.
  • Provide therapeutic interventions, supportive counseling, and community outreach to address high-risk situations and complex interpersonal concerns.
  • Offer education and guidance on healthcare consent and capacity, as well as financial and legal matters.
  • Support visitors requiring hospitalization and participate in advanced care planning and goals of care discussions.
Qualifications
  • Bachelor of Social Work (BSW) or Master of Social Work (MSW) degree required.
  • Current registration and membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Experience in acute care hospital settings, with a strong focus on clinical skills, assessment, counseling, and discharge planning.
  • Strong knowledge of community resources and ability to work collaboratively with community partners.
  • Familiarity with relevant legislation, including the Mental Health Act, Consent to Treatment Act, and Healthcare Consent Act.
  • Demonstrated ability to maintain an evidence-based practice approach and commitment to professional development.
