Community Health Workers play a vital role in our communities, providing essential care that significantly enhances the quality of life for clients and patients.

As a Community Care Assistant at Vancouver Coastal Health, your responsibilities will include:

  • Delivering home support services to clients under the guidance of the Supervisor/Coordinator/Manager or their delegate.
  • Assisting clients with various levels of care while observing and reporting any notable changes in their condition.
  • Collaborating with the healthcare team, clients, and their families to promote independence in daily living activities.
  • Encouraging and motivating clients to engage actively in their care.
  • Providing valuable input in the care planning process to the healthcare team.

New graduates from the Health Care Assistant program are encouraged to apply.

Qualifications

Education & Experience

  • Completion of a recognized Health Care Assistant Program.
  • Current/Active Registration with the BC Community Health Worker and Care Aide Registry.
  • Experience working with individuals with dementia, geriatric, and/or palliative populations is an asset.
  • A valid BC Driver's license and access to a vehicle may be required for local travel.

Knowledge, Skills & Abilities

  • Understanding of client-centered care and the promotion of independence.
  • Demonstrates proper body mechanics and knowledge of injury prevention.
  • Ability to operate mechanical lifts safely.
  • Familiarity with workplace violence prevention and personal safety techniques.
  • Knowledge of patient-centered care, particularly for individuals with dementia.
  • Ability to practice from a harm reduction perspective.
  • Basic technology skills and the ability to use a computerized client care information system.
  • Proficiency in verbal, nonverbal, and written communication through various methods.
  • Understanding of self-care and motivational support principles.
  • Ability to provide culturally safe and competent care, especially when working with Indigenous peoples.
  • Skill in organizing workload priorities within a team setting.
  • Ability to follow directions from the healthcare team and clients.
  • Competence in performing home management tasks.
  • Capacity to motivate clients and support their progress towards therapeutic goals.
  • Ability to respond effectively in a workplace environment.
  • Physical capability to fulfill the duties of the position.

Additional Information

Work hours, including days off and work areas, may be subject to change based on operational needs and the provisions of the Collective Agreement and applicable regulations. Successful candidates may need to undergo a Criminal Records Review Check.
