About the Job:

Job Summary:
As a Transition Liaison Registered Nurse with Interior Health Authority, you will play a critical role in ensuring seamless patient care transitions between healthcare facilities and services across the continuum of care. This is a RELIEF FULL-TIME opportunity that requires a skilled and compassionate nurse to coordinate patient access and care activities.

Key Responsibilities:
• Provide comprehensive nursing care to patients, working independently and as part of an interdisciplinary team to enhance quality of life and promote patient-centered care.
• Collaborate with healthcare providers, patients, and families to assess patient needs and eligibility for services and resources, ensuring timely and effective care transitions.
• Develop and implement evidence-based practice in nursing care, maintaining and advancing clinical competence to deliver high-quality patient care.
• Plan, implement, and evaluate patient access and care transition activities, identifying variances and resolving patient, caregiver, and system issues.
• Establish and facilitate communication linkages between healthcare facilities, services, community partners, physicians, and patients and their families to ensure coordinated care.
