Job Summary:
The Healthcare Transition Liaison Registered Nurse plays a vital role in ensuring seamless patient care transitions between Interior Health facilities and healthcare services across the continuum of care. This is a RELIEF FULL-TIME opportunity until further notice.
Key Responsibilities:
• Provides comprehensive nursing care to patients, working independently and as part of an interdisciplinary team to enhance quality of life and address patient needs.
• Collaborates with healthcare providers to assess patient needs, identify variances from care plans, and resolve patient, caregiver, and system issues.
• Establishes and facilitates communication linkages between healthcare services, community partners, physicians, health professionals, and patients and their families.
• Communicates changes in patient access and care transition service needs to relevant stakeholders.

This role requires a skilled and compassionate Registered Nurse who can work effectively in a fast-paced environment, prioritizing patient needs and ensuring timely care transitions.
