Long-Term Care Registered Practical Nurse (RPN)

At Southbridge Care Homes, we are committed to delivering high-quality care to our residents. We are seeking a compassionate and skilled RPN to enhance our team and positively impact the lives of those in our long-term care facilities.

Key Responsibilities:

  • Oversee and execute best practices within the Resident Assessment Instrument-Minimum Data Set (RAI-MDS) framework
  • Collaborate with an interdisciplinary team to ensure comprehensive resident care
  • Facilitate training and educational sessions regarding RAI MDS
  • Maintain exceptional standards of resident care while adhering to established protocols
  • Monitor, document, and report vital signs and health indicators
  • Organize and manage activities related to resident care
  • Support the administration and oversight of nursing personnel

Qualifications:

  • 3-5 years of experience in Long-Term Care settings
  • Active Registration with the College of Nurses of Ontario (CNO) in good standing
  • Proficient knowledge in nursing rehabilitation and collaboration with physiotherapy
  • Strong analytical and evaluative skills
  • Capability to lead training initiatives for adult learners
  • Familiarity with the InterRAI suite of assessment tools and patient classification systems
  • Excellent communication abilities
