Job Summary

The Mount Carmel Clinic is seeking a skilled and compassionate Community Outreach Specialist to join our team. As a key member of our coordinated service delivery team, you will work proactively with new and existing clients, offering a range of support and recovery services. Your primary focus will be on building relationships, advocating for clients, and providing services in a non-judgmental and culturally appropriate manner.

Key Responsibilities

  • Develop and maintain relationships with clients, community organizations, and other stakeholders to ensure effective service delivery.
  • Provide outreach services to hard-to-reach populations, including those with complex needs and barriers to access.
  • Collaborate with interdisciplinary teams to ensure comprehensive and coordinated service delivery.
  • Conduct assessments, develop service plans, and provide support and recovery services to clients.
  • Participate in ongoing professional development to enhance knowledge and skills in areas such as relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution.

Requirements

  • Minimum of 2 years' experience working in community, preferably with the program's target population.
  • Experience providing outreach services to hard-to-reach populations.
  • Valid Drivers' license and access to a reliable vehicle.
  • Post-secondary education in a Social Services field.

Preferred Qualifications

  • Knowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families.
  • Proven ability to work within an interdisciplinary team environment with respect, professionalism, cooperation, sensitivity, and cultural awareness.
  • Strong knowledge, commitment, and demonstrated ability to apply diversity, equity, cultural safety, pro-choice, social determinants of health, population health, harm reduction, trauma-informed, and community development principles in all aspects of the work.
