Job Summary

The Winnipeg Regional Health Authority is seeking a skilled Community Outreach Specialist to join our team. As a Community Outreach Specialist, you will work with clients affected by systemic marginalization and substance use who have complex health and social needs. Your primary responsibility will be to provide support and recovery services to clients, working proactively with new and existing clients to help them build new skills and achieve their goals.

Key Responsibilities

  • Provide outreach services to clients in a non-judgmental and culturally appropriate manner
  • Work with clients to develop and implement service plans that meet their unique needs
  • Collaborate with other service providers to ensure a coordinated approach to client care
  • Provide education and support to clients on health and wellness topics
  • Conduct home visits and other outreach activities as needed

Requirements

  • Minimum of 2 years' experience working in community, preferably with program target population
  • Experience providing outreach with hard to reach populations
  • Proven knowledge and ability in critical thinking, decision-making, problem-solving and conflict-resolution skills
  • Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes
  • Commitment to the principles that guide the Winnipeg Regional Health Authority, as reflected in the history, vision, mission and values
  • Knowledge and historical factors that affect the health and well-being of Indigenous individuals and families
  • Proven ability to work within a team environment with respect, professionalism, cooperation, sensitivity and cultural awareness
  • Strong knowledge of diversity, equity, cultural safety, pro-choice, social determinants of health, population health, harm reduction, trauma-informed and community development approaches

Education and Qualifications

  • Post-Secondary Education in a Social Services field
  • Valid Drivers' License and access to a reliable vehicle
