Job Summary

The Mount Carmel Clinic is seeking a skilled and compassionate Community Outreach Specialist to join our team. As a key member of our coordinated service delivery team, you will work proactively with new and existing clients, offering a range of support and recovery services. Your primary focus will be on building relationships and providing services in a non-judgmental and culturally appropriate manner.

Key Responsibilities

  • Develop and maintain relationships with clients, understanding their needs and goals
  • Provide support and recovery services, working within existing service plans and on an as-needed basis
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support
  • Utilize critical thinking, facilitation, and problem-solving skills to address client needs
  • Communicate effectively with clients, colleagues, and stakeholders

Requirements

  • Minimum of 2 years' experience working in community, preferably with the target population
  • Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills
  • Excellent verbal and written communication skills and proficiency in the use of computer systems
  • Post-secondary education in a Social Services field (an asset)
