Position Overview
CarePartners is initiating a new program in collaboration with Home and Community Support Services (HCCSS) Central East, seeking dedicated community-based support workers (CSWs) to participate in the High Intensity Supports at Home (HISH) initiative. These proficient CSWs will engage with patients and their families to facilitate care during transitions within the healthcare framework. The HISH program emphasizes Comprehensive Care, ensuring that all team members, including Nursing Therapy, Personal Support, Social Work, and Community Support Services, collaborate effectively to address the patient's needs. Selected CSWs will undergo specialized training and certification in Gentle Persuasive Approach (GPA).
Why This Role is Unique
This innovative staffing model allows HISH CSWs to enjoy stable work hours consistently, while also becoming integral members of a larger healthcare team committed to delivering high-quality patient care and enhancing the transition experience for both patients and their caregivers.
The work schedule consists of 7, 12-hour shifts over a 2-week rotation, including weekends :

This totals seventy-seven (77) hours for each bi-weekly pay period

Actively participating as a valued member of the interdisciplinary care team Assisting with daily living activities, including personal care, household tasks, respite care, medication support, and special functions Ensuring a safe environment by applying proper body mechanics, safe transfer techniques, and home safety practices Offering cognitive, social, and emotional support to patients Preparing nutritious and varied meals, including special diets as outlined in care plans, while adhering to food safety and hygiene standards Providing updates on patient status and engaging in patient/family care planning meetingsQualifications
A strong commitment to positively impacting the lives of patients Excellent interpersonal skills Ability to work independently and collaboratively within a team Experience with patients having complex care needs, such as dementia and mental health challenges A valid driver's license and insured vehicle Relevant education as a Community Support Worker (e.g., Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or current enrollment in a Nursing program), with preference for candidates holding a PSW certificate or equivalent To ensure patient safety, a current vulnerable sector check and clear background check are requiredAdditional Opportunities
New Ontario PSW graduates may qualify for a * $10,000 bonus ** from an approved PSW program in Ontario, contingent upon a commitment to work full-time for 12 months, with payments at 6 and 12 months.
** Additional $10,000 relocation bonus ***
*** Available for those relocating to rural, remote, or northern areas, with a commitment to work for 12 months, payable at the start and after 6 months.
