About the RoleWe are seeking a highly skilled and compassionate Nurse Practitioner to join our Geriatric Assessment and Intervention Network (GAIN) team. As a Geriatric Care Specialist, you will be responsible for providing comprehensive geriatric assessments, developing and implementing care plans, and providing direct patient care to frail seniors living at home.
Key Responsibilities- Conduct comprehensive geriatric assessments, including client interviews and physical examinations, to identify client needs and develop care plans.
- Provide direct patient care, focusing on health promotion and maximizing client safety and function.
- Develop and implement goals and treatment interventions for complex case situations or specialized problems.
- Liaise with other services and team members to coordinate the interdisciplinary plan of care and facilitate the efficient movement of clients through the healthcare system.
- Monitor clients' goals and outcomes and initiate the planning process.
- Baccalaureate in nursing or equivalent from an accredited university (Master's degree in nursing preferred).
- Nurse Practitioner certification through an accredited institution.
- Current Certificate of Competence with the college of Nurses of Ontario.
- Geriatric training and certification, such as P.I.E.C.E.S., Montessori, and G.P.A.
- Minimum 3-5 years related clinical and/or management experience, preferably in a community healthcare setting.
