RhynoCare is currently in search of a dedicated and skilled individual to fill the role of Nurse Practitioner (NP) within a Long-term Care (LTC) environment.

Key Responsibilities:

  • Provide comprehensive primary health care to residents in LTC, focusing on the management of both acute and chronic health issues, as well as health promotion and disease prevention.
  • Offer on-call support as required by the operational needs.
  • Conduct thorough clinical evaluations, utilizing sound clinical judgment and diagnostic skills within the full scope of practice as outlined by the regulatory body.
  • Lead clinical initiatives within the LTC team, addressing resident care needs including admissions, discharges, and end-of-life care.
  • Utilize various assessment tools to evaluate health status based on individual resident requirements.
  • Gather complete health histories that encompass physical, psychosocial, emotional, cultural, and ethnic factors.
  • Perform physical examinations, interpret findings, and order necessary diagnostic tests, applying evidence-based clinical reasoning.
  • Engage with residents and their families to create personalized care plans, synthesizing health information to establish diagnoses and identify health risks.
  • Communicate assessment results and treatment plans effectively to residents, employing knowledge of relevant medical disciplines.
  • Select appropriate interventions from a range of therapeutic options to support residents' health and well-being.
  • Ensure safe resident care by proactively addressing potential risks and adverse events.
  • Coordinate with discharge planning services to facilitate smooth transitions for hospitalized residents returning to LTC.
  • Collaborate with community health leaders to develop and assess systems that enhance continuity of care.
  • Participate actively in quality assurance programs to maintain professional competence.
  • Allocate 80% of work time to direct resident care and 20% to research and educational initiatives aimed at improving care standards.
  • Demonstrate cultural competence and knowledge of health determinants in assessments and treatment plans.
  • Exhibit leadership capabilities within an inter-professional team, fostering effective relationships and providing mentorship.
  • Build and maintain collaborative partnerships with diverse stakeholders and work effectively within multidisciplinary teams.
  • Possess strong verbal and written communication skills.
  • Be flexible in scheduling, including potential evening or weekend shifts, and participate in after-hours coverage.
  • Familiarity with best practice guidelines and relevant regulations is essential.

Essential Qualifications:

  • Registered with the College of Nurses of Ontario in the Extended Class.
  • Bachelor's degree in Nursing (BScN); a Master's degree in Nursing (MSN) with a Nurse Practitioner specialization is preferred.
  • Membership with the Nurse Practitioners' Association of Ontario (NPAO) is an asset.
  • Professional liability insurance is required.
  • National certification in gerontology is advantageous.
  • Proficient in crisis management and effective communication.
  • Strong report writing skills for clinical documentation.
  • Ability to engage clients in a supportive and understanding manner.
  • Current Basic CPR certification is mandatory.
  • Valid Vulnerable Sector Check (VSC) is required.
  • Compliance with immunization requirements as mandated by health authorities.
