Position Overview

Key Responsibilities

  • Conduct assessments and implement SLP interventions (e.g., swallowing assessments, dysphagia management, communication support) in a home-based environment
  • Utilize a collaborative, interdisciplinary team approach to service delivery
  • Support a holistic and restorative care model (e.g., enhanced stroke pathway, cognitive communication supports)
  • Leverage digital health platforms for effective care delivery, collaboration, documentation, and intervention

Qualifications for Success

  • Registered member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
  • Commitment to person and family-centered care
  • Proficient in delivering care both in-person and through virtual methods
  • Demonstrated passion, empathy, sound judgment, and critical thinking in client interactions
  • Excellent communication skills and ability to collaborate within an interdisciplinary team while also working independently
  • Flexibility and strong critical thinking skills applicable to various situations
  • Appreciation for the role of a therapist in supporting clients to remain in their homes
