Key Responsibilities (in-person or virtual delivery)

  • Conduct assessments and implement SLP interventions (e.g., swallowing assessments, dysphagia management, communication) in a home-based environment
  • Utilize a collaborative, interdisciplinary team approach to care
  • Support a holistic and restorative care model (e.g., enhanced stroke pathway; cognitive communication supports)
  • Leverage digital health platforms for care delivery, collaboration, documentation, and intervention

Qualifications for Success

  • Registered member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
  • Commitment to person and family-centered care
  • Comfortable delivering care in-person and through virtual methods
  • Demonstrated passion, empathy, sound judgment, and critical thinking in client decision-making
  • Excellent communication skills and ability to collaborate within an interdisciplinary team as well as work independently
  • Flexibility and proven critical thinking skills applicable to various situations
  • Appreciation for the therapist's role in enabling clients to remain at home
