The Care Model

Key Responsibilities (in-person or virtual delivery)
- Conduct assessments and implement SLP interventions (e.g., swallowing assessments, dysphagia management, communication support) in a home-based environment
- Utilize a collaborative, interdisciplinary team approach to care
- Support a holistic and restorative care philosophy (e.g., enhanced stroke pathways, cognitive communication supports)
- Leverage digital health platforms for care delivery, collaboration, documentation, and intervention
Qualifications for Success
- Registered member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
- Commitment to person and family-centered care
- Comfortable delivering care both in-person and through virtual methods
- Passionate, empathetic, and skilled in critical thinking for client decision-making
- Excellent communication skills and ability to collaborate within an interdisciplinary team while also working independently
- Flexible and adept at applying critical thinking to diverse situations
- Appreciation for the therapist's role in supporting clients to remain in their homes
