About the Role:

This community nursing position offers a unique opportunity to provide care and support to patients in their own homes. As a Licensed Practical Nurse (LPN), you will be responsible for delivering high-quality patient care, working collaboratively with patients and their families to develop and implement personalized care plans.

Key Responsibilities:

  • Conduct scheduled visits in the community to assess patient needs and provide care within your scope of practice
  • Provide safe and effective nursing care, using appropriate assessment tools and techniques to identify patient needs
  • Develop and implement care plans, monitoring patient progress and making adjustments as required
  • Build strong relationships with patients and their families, using a collaborative approach to care
  • Maintain accurate and up-to-date patient records, documenting patient encounters and care plans

Requirements:

  • Registration with the NSCN as a Licensed Practical Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in delivering community care is an asset
  • Reliability and flexibility, with a willingness to travel to and from patient residences within a designated area
  • Experience working with vulnerable populations
  • Passion for making a difference in your community
